Medical record requests: PDP's health-record request contact is pdpmedicalrecordrequests@yescarecorp.com.
Formal public-record requests: use the City of Philadelphia Open Records Policy and the statewide Right-to-Know Law request form. The City policy requires enough detail to identify the records and provides the Pennsylvania RTKL response framework.
Use these routes based on the type of record or custody status involved:
- For a person in the Philadelphia jail system, start with jail inmate records and the Philadelphia Incarcerated Person Locator, then call Classification, Movement, and Registration if the result is unclear.
- For booking photos, review jail roster mugshots and use the City RTKL process when a photo is not displayed in the public locator.
